Billing & Insurance
We accept most of Minnesota’s major health insurance plans. Please check with your individual health plan for the most current information about our status as a provider with your plan.
The following insurance companies are contracted with Southdale ObGyn Consultants, P.A.:

- Blue Cross Blue Shield of MN
- Minnesota Medical Assistance
- Preferred One
- United Health Care
You will need to present your insurance card at each appointment. If your insurance has changed since your last appointment, please be sure to let us know.
Referrals: Many insurance plans may not pay for procedures or visits that are recommended or ordered by your health care provider. Only your primary care clinic can provide medical referrals. Please be sure that you are familiar with what your insurance plan referral policy is prior to your visit. If referrals are required, always secure your referral prior to scheduling specialty procedures.
Coding for Services: All health care providers bill for services using billing codes. These codes tell the insurance company what was done and why. Routine and preventive services cause confusion for many patients. It is not uncommon for patients in the course of a visit to receive both treatment for a problem and preventive service. When this occurs, we are required to charge for both services.
Copays are due at the time of service. It is your responsibility to know which services require copays. If you are unsure of your copay requirements, please contact your health plan.
Payment is due upon receipt of your first billing. A finance charge of 6% will accrue monthly on patient balances that are over 30 days old. If for some reason you cannot meet this requirement, please contact our business office for payment arrangements.
Patients with no insurance coverage are required to pay $500 at the beginning of each clinic appointment and the balance will be charged and payable at the end of the visit. Surgical procedures will need to be prepaid.
Copays must be paid to the patient representative at the time of your appointment.
Our credit policy applies even while insurance coverage is pending.
VISA/MasterCard/Discover/American Express is accepted for payment.
